Glycemic treatment strategies
Pharmacological studies evaluated tirzepatide, the oral GLP-1 receptor agonist lotiglipron, and vildagliptin; tirzepatide produced normoglycaemia in 553 of 912 Japanese participants after 52 weeks, lotiglipron was tested in placebo-controlled phase 1 studies, and vildagliptin lowered fasting glucose and HbA1c in patients with diabetic kidney disease. Kidney and cardiac complications
Studies of diabetic kidney disease used a biopsy-proven cohort, biomarkers, and experimental models: urinary renal tubular epithelial cells and casts were evaluated as predictors of renal outcomes, circulating 8-hydroxy-2'-deoxyguanosine was higher in patients with diabetic kidney disease, and acteoside was tested for effects on oxidative injury, autophagy-lysosome dysfunction, and renal fibrosis. (PMIDs 39031241, 39271385, 38852699) Experimental treatments for diabetic nephropathy included human umbilical cord mesenchymal stem cells, which inhibited ferroptosis through the JNK/KEAP1/NRF2 pathway in type 2 diabetic mice and high-glucose- and palmitate-stimulated human renal tubular epithelial cells. (PMID 39602247) Cardiovascular studies assessed coronary microvascular dysfunction using angiography-derived microcirculatory resistance and evaluated cardiac structure and function with echocardiography, cardiac MRI, strain, perfusion, and tissue-characterization measures in people with diabetes. Diabetic wound repair
Preclinical wound studies addressed infection, oxidative stress, hypoxia, and impaired angiogenesis. In diabetic rats, photodynamic antimicrobial chemotherapy combined with an antibiotic improved angiogenesis, tissue maturation, and ulcer healing, while calycosin accelerated healing through reduced oxidative stress and activation of the Nrf2/HO-1 pathway. (PMIDs 38651324, 40963425) Biomaterial approaches included Bergenia ciliata-loaded hydrogels, gas plasma treatment, and glucose-responsive hydrogels containing calcium peroxide, catalase, and gallium-polyphenol nanoparticles. Bergenia ciliata-loaded hydrogels promoted wound contraction, reepithelialization, angiogenesis, and wound maturation; gas plasma improved superficial tissue oxygenation and stimulated processes involved in tissue repair; and the glucose-responsive hydrogel inhibited microorganisms, continuously released oxygen, and alleviated cellular hypoxia. Molecular and genetic heterogeneity
Genetic studies linked type 2 diabetes susceptibility and clinical heterogeneity to specific pathways: the P2XR4 rs25644 G allele was associated with diabetes risk in a case-control study, while pathway-specific polygenic scores in 18,217 Chinese patients from Hong Kong were used to examine associations with cardiometabolic profiles, age at onset, glycaemic deterioration, and renal and cardiovascular outcomes. (PMIDs 38954847, 39531041) A cross-sectional study in Congolese patients measured NOX2, IL-6, CRP, COX-2, and caspase-3 in relation to metabolic, inflammatory, and apoptotic markers, and another study investigated metformin-associated expression of DNMT1, TET1, OGG1, and AID in patients and insulin-resistant HepG2 cells. (PMIDs 41607455, 41087712) Metabolic-flux experiments identified glucosamine as a metabolite connecting elevated glucose with mTORC1 activation in pancreatic islets and kidney, using dapagliflozin or insulin treatment in diabetic rodents and showing that pharmacological inhibition of the glucosamine/mTORC1 axis alleviated Ξ²-cell stress, improved glycaemic control, and restored Ξ²-cell function. Monitoring and risk prediction
Continuous glucose monitoring was used both to personalize exercise timing in adults with established type 2 diabetes and to identify time-above-range thresholds associated with later diabetes onset in people without diabetes. (PMIDs 40023292, 40449624) Diabetes-related vascular and retinal changes were assessed with optical coherence tomography angiography, electroretinography, toe photoplethysmography during local heating, and arterial-stiffness measurements linked to retinal, corneal, and peripheral neuropathy.
